I don't keep a GHB in my car because I drive a hatchback (without a trunk cover thing in order to accommodate my dog) and the appearance of anything valuable in my car would pretty much guarantee the car gets broken in to, no matter what part of the city I'm in.
I keep a small emergency kit in the glove box: mini FAK, leatherman, pens, matches & lighters, and a few of those Coleman emergency blankets and rain ponchos.
In the trunk, with the spare tire, I have a little tool set, some leather gloves, and duct tape.
I keep a fairly well-stocked GHB under my desk at work. Its contents are designed primarily to get me home in the event I had to walk the seven or so miles from work to home, or to keep me comfy if I have to bug out from work to a friend's place or hotel or whatever.
Partial contents of the GHB: towel, washcloth, 3 pair socks, 3 pair underwear, 1 pair khaki pants, 2 shirts, mini travel-size toiletries I got at target for $1 each (deodorant, toothpaste, razor, toothbrush, shaving cream, soap), mini FAK, 2 liters water, and a little bit of food - some Cliff bars, fig newtons, and beef jerky. Also, a flashlight and a swiss army knife, work gloves, a few plastic bags, and some paracord.
All this is contained in a gym bag I got at Target for a couple of bucks a few years ago. The bag won't stand up to much abuse, but I'm pretty sure it'll get me where I'm going.
I have a messenger bag in my closet at home with substantially similar contents in the event I ever have to bail out of my house without having time to pack.

Mine's on the borderline between a GHB & a BOB. It's also my EDC. (BOB= Bug out Bag, EDC= Every Day Carry)
On my person: Ammo for what's in the glovebox, large pocketknife, BIC lighter, Surefire G-2 Nitrolon, cell phone. At work, I'll have my 2GB thumb drive, & my badge holder with my work ID and some extra cash.
Bookbag, front pocket: Holster for what's in the glove box, spare magazines or speedloaders for what's in the glove box, roll of gaffer's tape, earplugs, spare keys for truck, folding titanium spork, folding utility razor, BIC lighter, matchbook, firesteel, Energizer 2in1 flashlight (incandescent directional & fluorescent area lamp in one unit), assorted pens & markers, photocopy of the back of my work ID (has the barcode to get in past the office scanlocks), $5-10 in quarters, small to large USB cable for charging my cell phone. Need to replace the glowsticks that used to be in there.
Bookbag, main pocket: Um, kindling. Actually, it has gaming books & my work tablet computer. Except for a notebook, the books would be ditched to save weight & room. The tablet & charger are useful as a communication resource in anything short of an EOTWAWKI.
Bookbag, back pocket: 3 MRE entrees & side items, 2 liter camelback-type water bladder, water purification tablets, hand-forged utility knife, short prybar, compass, binoculars, AM/FM/Shortwave radio in a crush & water proof box, 2 spare CR123 batteries for the Surefire, 4 spare AA batteries in a carrier for the radio or the flashlight, spindle of paracord, 2 hotel room coffee condiment packs (napkins, sugar, creamer, swizzle stick), two packets of MRE bogroll.
In the glovebox: Either a M1911A1, CZ-52, or a Smith & Wesson Model 686, spare pocketknife, half a dozen glowsticks.
Back seat of the extended cab: Machete, camo tarp, swiss army poncho, trauma first aid kit in a lunch box type cooler bag.
Trauma First Aid Kit, front pouch: ziplock bag of assorted band-aids, ziplock bag of 2", 3", & 4" gauze pads, ziplock bag of assorted wipes (alcohol, betadine, etc. Need to add single-serving sunburn creams to it), two rolls medical tape, one partial tube neosporin, tweezers, folding utility razor, flashlight, pen-shaped pump container of alcohol hand sterilizer.
Trauma First Aid Kit, main pouch: 4" Israeli trauma dressing, 4" Olaes trauma dressing, 5 rolls kerlex, box of BC headache powder, box of Benadryl, partial box of Immodium, five pairs of nitrile gloves, 5 packets of Quik-Clot Sport, Asherman Chest Seal, 2 glow sticks, 2 glucose shots, medical tape, Coban medical wrap. Probably a thing or two I've forgotten, but the kit's out in my truck & not here at my desk. Speaking of which...
Desk Drawer @ Work: 4 MRE entrees and 2 MRE sides, two ramen cups, ziplock bag of tea & sugar packets, can of coffee & chicory, Melita coffee drip widget which goes on top of any mug, coffee filters enough to last 'til doomsday, spare flashlight, random plasticware from the cafeteria.
ETA: Forgot all about the SAM Splints. Got a finger sized one in the front pocket, and a 36" standard size one in the main compartment. I've also got a 9" standard size one on order. Should be here next week along with another Olaes dressing and some poison ivy wipes. _________________ "If it ain't the Devil's music, you ain't doin' it right" - Chris Thomas King

As far as the book bag goes, no. Unless I'm at work, a coffee shop, or a friend's place, the bag either stays in the truck (if I'm heading to or coming from one of those other places) or stays at home. _________________ "If it ain't the Devil's music, you ain't doin' it right" - Chris Thomas King

My EDC is a small backpack and so far I've carried it into Wal-mart and the movie theater w\o any problems. I always carry it with me if I am going to be more than a few yards from my vehicle. _________________ "If this ain't a mess, it'l do 'till one shows up"
